your insurance company has "Out of Network Benefits", you as a patient
or family member may submit a copy of your bill for a percentage
reimbursement of the fees paid based on your insurance plan's policy.
All fees will be collected in full at the time of your service; payable
by cash, credit card, or cashiers check. |
